Diabetes and high blood pressure are the leading causes of kidney disease. If you or a loved one have either of these conditions, you are at risk for developing kidney disease.

Kidney damage happens slowly, over time, but the good news is that if caught early, you can make changes to slow or stop the progression of kidney disease.

That's where Path to Wellness can help! Our comprehensive screening tests for diabetes, high blood pressure, kidney disease, and other risk factors that can contribute to kidney disease.

The screening is FREE and test results are provided on-site in about an hour! Screening participants have the opportunity to speak one-on-one with a volunteer clinician who will help them understand their screening results and make recommendations for next steps.
