Path to Wellness - Free health screening! (Ages 18 and older)

Did you know that diabetes and high blood pressure are the top two causes of kidney disease? If you or a family member have either (or both!) of these conditions, you are at an increased risk of developing kidney disease.

Our free community health screening will provide you with on-site lab tests for diabetes, cholesterol, triglycerides, and kidney function. You'll also receive screenings for blood pressure, and physical measurements (height, weight, BMI, and waist circumference). Results are available within about an hour and each participant will receive a one-on-one free consultation with a clinician to help them understand their screening results.

Community resources to help access medical insurance and primary care services will also be available.
